include: one folder per subsystem, and includes that name it

Sixty-five headers sat side by side in include/, so `#include "irq.h"`
said nothing about where irq.h was or what it belonged to, and errno.h,
stat.h and fcntl.h at the top shadowed the system's for any host tool
built with -I./include. They are in folders now -- arch/ core/ fs/
dev/ gfx/ net/ uapi/ (and ldls/ for the Linux driver layer) -- and
every include names the folder, as Linux's do: "core/proc.h".

Done mechanically and checked the hard way: the kernel built from this
tree disassembles identically to the one built from the tree before
it. The full image boots, and userland is the same set of programs.
Paths in docs and comments follow; docs/guides/layout.md has a table
of what is where, and tools/fix-includes.py rewrites old-style
includes from the folders in the tree, for code written before this.

// Drawing, and the text console under it, on whatever display is bound.
//
// This was kernel/dev/vga.c, and every consumer called it by that name:
// panic.c drew the crash screen with vga_fill_rect, the console printed with
// vga_putc, /dev/fb0 handed out vga_fb_phys(). None of that code has anything
// to do with a VGA -- it is bitmap drawing over a linear framebuffer, and the
// only VGA-specific part was where the framebuffer came from.
//
// So the driver pushes its scanout in here (gfx_attach_scanout) and everything
// above draws through these calls. A second display driver becomes a second
// caller of one function rather than an edit to the crash screen. This is the
// same arrangement as include/dev/netdev.h, for the same reason.
//
// Not the same thing as the display class in include/gfx/display.h. That is the
// compositor's interface -- acquire a back buffer, present a rectangle, wait
// for vblank -- and it is about *frames*. This is the kernel's own drawing
// surface, used by the console and the crash screen, which do not double
// buffer and must keep working when nothing else in the system does.

// A QR encoder, for getting a crash report off a machine whose network stack is
// dead.
//
// A panic report is worth nothing if it cannot leave the machine, and at panic
// time there is no network, no filesystem write that can be trusted, and often
// no second machine on the desk. A phone camera is a working out-of-band
// channel that needs no drivers on our side at all. See docs/architecture/graphics.md.
//
// Deliberately small, in three ways:
//
// - Byte mode only. The report is arbitrary bytes; alphanumeric mode would
// pack tighter but only for a restricted character set.
// - Error correction level L only. The screen is a clean, evenly lit,
// high-contrast source -- the failure mode QR's stronger levels protect
// against (dirt, print damage, curved packaging) does not apply.
// - Versions 1 to 6 only. Version 7 is where QR starts requiring an extra
// 18-bit version-information block in two more places on the symbol, and
// version 10 is where the character count field grows and the alignment
// pattern grid becomes 2-dimensional. Stopping at 6 avoids both, and
// 134 bytes is a large crash report.
//
// If a payload ever needs more than 134 bytes, the honest fix is to shorten the
// report -- not to reach for version 10 and its two extra tables.

// Text rendering.
//
// The console used to draw text by indexing `font_data[256][16]` and stepping
// the cursor eight pixels. That is a *character cell grid*, and it was the
// thing in the way -- not the font. A grid like that cannot do variable-width
// glyphs, cannot position text anywhere except on a multiple of eight, cannot
// anti-alias, and cannot represent a codepoint above 255. Swapping the bitmap
// for a nicer bitmap changed none of that.
//
// None of it is true any more, and the console draws through this file. What
// remains a grid is `vt.c`'s model of a terminal, which is not the same claim:
// a terminal *is* rows and columns -- VT100 escapes address them and
// TIOCGWINSZ reports them -- and it reaches this renderer by handing it one
// cell at a time, as a surface.
//
// So this is a layer, not a font. Three things it fixes:
//
// *Glyphs have metrics.* A glyph knows its own size, its bearing and how far
// to advance afterwards. That is what variable-width text is; everything else
// follows from it.
//
// *It draws into a surface, not into the screen.* A compositor draws into an
// off-screen buffer and presents it. Rendering that writes straight to the
// VGA aperture is unusable to it, which is why this takes a target.
//
// *The font is behind an interface.* The bitmap font is one implementation.
// A TrueType rasteriser is another, and arrives without any caller changing:
// glyphs carry 8-bit coverage, so an anti-aliased outline renderer already
// has somewhere to put its output.
struct font;
// Somewhere to draw. Deliberately not a `struct device` or a framebuffer: the
