Initial commit: Schunix as it stands before the SchOS rename

An x86-64 operating system built from xv6, now largely rewritten: SMP,
a copy-on-write root filesystem (SchFS), PCI/USB/virtio drivers, a TTF
rasteriser, and a Linux-numbered syscall ABI.

No history before this point -- the tree was never under version control.
This commit exists so the rename that follows has something to undo to.

// The device model. See include/device.h for why it exists.
//
// A fixed table rather than a linked list: this is populated once during boot
// by whichever bus found each device, and read afterwards. Nothing is removed
// yet -- hot-unplug would need it, and when that arrives the in_use flag is
// already the hook for it.
//
// No lock. Every device_add() happens during single-threaded boot, before
// userspace exists, and every reader afterwards only walks. If a bus ever
// enumerates at runtime -- USB hotplug is the obvious one -- this needs a
// spinlock, and the reader in procfs needs to take it.
